Job summary

Care Lync is seeking a dedicated Support Coordinator/Service Coordinator/Case Manager to develop individualized care plans, coordinate services, and monitor outcomes to support adults with IDD, Autism, and related disabilities in the Columbia area.

The role emphasizes helping clients access housing, medical, social, and community services while maintaining confidentiality and compliance with program regulations.

Qualifications

• BS/BA degree in related course work.
• Minimum of 1 year(s) related experience providing case management for individuals with IDD or disabilities.
• Experience with Microsoft Office, state-maintained databases.
• Maintain CPR/First Aid certification and updated state related Clearances.
• Valid Driver's License including personal vehicle insurance coverage for liability and current vehicle registration.

Responsibilities

• Complete assessments to identify participant needs and desires for service delivery.
• Develop Person-Centered Service Plans and update annually or after changes.
• Ensure participant choice of providers by offering information to make informed decisions.
• Coordinate with care coordinators to ensure seamless service delivery.
• Travel up to 50% to participant locations as needed.
