Job summary

Care Lync is seeking a dedicated Support Coordinator/Service Coordinator/Case Manager to develop individualized care plans, coordinate medical, housing, and social services, and monitor outcomes to help adults with IDD and autism live independently.

The role emphasizes brokered services, confidentiality, and collaboration with a consolidated care team to ensure professional, cost-effective support. Travel up to 50% is required.

Qualifications

• BS/BA degree in related coursework.
• Experience with Microsoft Office, state-maintained databases.
• Maintain CPR/First Aid certification and updated state-related clearances.
• Maintain training/certifications sponsored by the company.
• Valid driver’s license with personal vehicle insurance and current registration.

Responsibilities

• Complete assessments to identify participant needs and desires for service delivery.
• Develop a Person-Centered Service Plan and modify annually or with significant changes.
• Ensure participant can choose providers with informed information.
• Coordinate with care teams to align ISP with behavioral and physical care.
• Report incidents of abuse, neglect or exploitation to supervisor.
• Travel to participants up to 50% to coordinate services and supports.
