Job summary

Care Lync is seeking a Service Coordinator to develop and implement individualized care plans for adults with IDD and related disabilities. The role focuses on coordinating services, ensuring ISP alignment, and monitoring outcomes to support independent living in home and community settings.

The coordinator will gather participant needs, connect with providers, and document activities while maintaining confidentiality and compliance with program regulations.

Qualifications

• BS/BA degree in related course work.
• Minimum of 3 years related experience providing case management.
• Experience with Microsoft Office and state databases.
• Ability to perform arithmetic with common units and decimals.
• Maintain CPR/First Aid certification and updated Clearances.
• Valid Driver's License and vehicle insurance coverage.

Responsibilities

• Complete assessments to identify participant needs and desires.
• Develops a Person-Centered Service Plan and updates annually.
• Ensure participant/providers choose informed options for ISP.
• Coordinate with care teams to align physical, behavioral and support services.
• Maintain accurate documentation and reporting.
• Travel to participant locations up to 50% as needed.
