Job summary

Care Lync is seeking a Support Coordinator / Service Coordinator to develop and implement individualized care plans for adults with intellectual, developmental, and physical disabilities within the Medicaid waiver framework. The role emphasizes coordinating housing, medical, social and community services to help clients live independently.

The position requires a BS/BA, at least 3 years of case management experience, and proficiency with Microsoft Office and state databases, with up to 50%

Qualifications

BS/BA degree in related course work.
Minimum of 3 years related experience providing case management for individuals with Intellectual Disabilities, Autism Spectrum Disorders, or Physical Disabilities.
Experience with Microsoft Office, state-maintained databases.
Maintain CPR/First Aid certification and updated state related Clearances.

Responsibilities

Complete assessments to identify participant needs for service delivery.
Develop Person-Centered Service Plans and update annually or with significant changes.
Coordinate with providers and care coordinators to ensure seamless services.
Ensure confidentiality of sensitive information.
Attend required trainings and recertifications.
Travel up to 50% to participants as needed.
