Job summary

Care Lync is seeking a dedicated Support Coordinator to provide case management and service coordination for adults with IDD, autism, and physical disabilities enrolled in Medicaid waivers. You will develop individualized plans and connect clients to housing, medical, social, and community supports.

In this role you will travel to assist participants, coordinate with care teams, monitor outcomes, and ensure services are delivered professionally and cost-effectively, while keeping client needs at

Qualifications

• BS/BA degree in related course work.
• Experience with Microsoft Office and state-maintained databases.
• Must complete and maintain training requirements.

Responsibilities

• Complete assessments to identify participant needs.
• Develops a Person-Centered Service Plan and update it as needed.
• Coordinate with providers and other care coordinators for seamless service delivery.
• Monitor and evaluate outcomes to ensure cost-effective services.
• Travel to participant locations as needed (up to 50%).
