Job summary

Care Lync in Maine is seeking a dedicated Support Coordinator / Service Coordinator to develop individualized care plans, coordinate services, and monitor outcomes for adults with IDD, disabilities, and autism enrolled in Medicaid waiver programs. The role requires travel to participants, proficiency with MS Office and state databases, and adherence to privacy standards.

You will partner with supervisors to align services with ISP goals and community supports.

Qualifications

• BS/BA degree in related field required or equivalent
• Experience using Microsoft Office and state-maintained databases
• Maintain CPR/First Aid certification and state required Clearances
• Valid Driver's License and personal vehicle insurance required
• Complete trainings and certifications as required by program regulations

Responsibilities

• Complete assessments to identify participant needs and desires for service delivery.
• Develop a Person-Centered Service Plan and update at least annually or with changes.
• Present provider options to participants to support informed choices.
• Ensure ISP is supported by Behavioral Support and Crisis Plans where applicable.
• Report any incidents of abuse, neglect or exploitation promptly to supervisor/director.
• Coordinate with other care coordinators and entities for seamless service delivery.
• Research and develop alternative solutions to meet participant needs.
• Maintain confidentiality of sensitive information per policy.
• Attend required trainings and recertifications.
• Communicate with supervisor about participant needs, abilities, and limitations.
• Travel to participant locations up to 50% as needed
