Job summary

Care Lync is a Social Services agency focused on Support Coordination for adults with Intellectual, Developmental and Physical Disabilities and Autism Spectrum Disorder who are enrolled in the Medicaid Waiver Program. Qualifications

• Bachelor's degree or higher in a related field.
• Minimum of 3 years in case management for disabilities.
• Proficiency with Microsoft Office and state-maintained databases.

Responsibilities

• Complete assessments to identify participant needs.
• Develop Person-Centered Service Plans and annual updates.
• Coordinate with providers to ensure ISP alignment.
• Maintain confidentiality and monitor outcomes.
• Travel to participants as needed (up to 50%).
