Job summary

Care Lync in Maine seeks a dedicated Support Coordinator to guide adults with disabilities and aging individuals through housing, medical care, and community resources. You will develop person-centered plans and coordinate services to promote independence.

Qualifications

• Bachelor's degree in Social Work, Psychology, Human Services, or related field.
• 3+ years of experience providing case management or support coordination to individuals with disabilities and/or aging adults.
• Strong organizational, communication, and advocacy skills.
• Comfortable traveling up to 50% to meet with participants in their homes and communities.

Responsibilities

• Conduct person-centered assessments to identify participants' needs, preferences, and life goals.
• Develop, implement, and regularly update ISP to ensure participants receive the right services.
• Provide participants with choice and advocacy, helping them make informed decisions about providers and care options.
• Coordinate with medical professionals, behavioral specialists, family members, and community resources to deliver high-quality care.
• Monitor and evaluate services to ensure safety, satisfaction, and progress toward independence.
• Maintain documentation, confidentiality, and compliance with program and ethical standards.
