Care Lync in Maine is seeking a devoted Support Coordinator to develop individualized care plans and coordinate services for adults with IDD, autism, and other disabilities enrolled in Medicaid Waiver. You will broker services, monitor outcomes, and help clients access housing, medical and community supports.

The role requires travel up to 50% and a BS/BA, proficiency in Microsoft Office and state databases, and a valid driver's license with insurance.

Competenze

BS/BA degree required or related field.
Experience with Microsoft Office and state-maintained databases.
Maintain CPR/First Aid certification and updated state related Clearances.
Valid Driver's License including personal vehicle insurance coverage.
Inform supervisor immediately when any change in auto insurance/policy, automobile registration and/or status of driver's license.

Mansioni

Complete assessments to identify participant needs and desires for service delivery.
Develop Person-Centered Service Plans and update annually or with significant changes.
Ensure participant choice of providers by giving information for informed decisions.
Coordinate with other care coordinators to ensure seamless service delivery.
Monitor and modify Behavioral Support Plan and Crisis Plan to align with ISP.
Ensure confidentiality of sensitive information and client rights.
Attend trainings and recertifications as required.
Communicate with supervisor about participants' needs and limitations.
Travel up to 50% to participants as needed.
