Job summary

Public Partnerships LLC (PPL) is seeking a Support Coordinator to educate, guide, and assist Medicaid participants in self-directed care. You will visit participants in their homes (up to 60 miles) and help them manage care plans, hire direct care providers, and maintain safe living environments.

You’ll split time between field visits and home-office documentation, collaborating with a dedicated team while using Microsoft Office, CRM tools, and electronic records to stay organized.

Qualifications

• A degree in social work, psychology, or related field preferred.
• Experience supporting vulnerable populations is a plus.
• Strong communication and organizational skills required.

Responsibilities

• Visit participants in their homes (up to 60 miles) to provide guidance and education.
• Assess home environments and help resolve safety concerns.
• Empower participants by helping manage employer responsibilities.
• Organize and manage caseload to maximize impact.
• Collaborate with a field team and use technology to stay organized.
