• Strong communication and interpersonal skills are essential.
• Valid driver’s license, reliable vehicle, and secure internet connection.

Responsibilities

• Educate and support participants in managing care plans and hiring providers.
• Visit participants in their homes (up to 60 miles) and assess safety.
• Document activities and coordinate with the home office.
