Job summary

Care Lync is seeking a dedicated Support Coordinator/Service Coordinator to manage individualized care plans and coordinate services for adults with intellectual and developmental disabilities. The role includes developing ISP-based plans, brokering services, and monitoring outcomes to ensure professional, cost-effective support.

Qualifications

• BS/BA degree required or related field.
• Minimum of 3 years related experience in case management for ID/Autism/Disabilities.
• Experience with Microsoft Office and state-maintained databases.
• Maintain CPR/First Aid certification and state clearances.
• Valid driver’s license and vehicle insurance on file.

Responsibilities

• Complete assessments to identify participant needs and desires for service delivery.
• Develop a Person-Centered Service Plan for each participant and update as needed.
• Ensure participant can choose providers with information to make informed decisions.
• Align Behavioral Support Plan and Crisis Plan with the ISP.
• Report all incidents of abuse, neglect, or exploitation immediately.
• Coordinate with other care coordinators to ensure seamless service delivery and travel up to 50%.
