Job summary

Care Lync is seeking a dedicated Support Coordinator to help adults with disabilities and aging individuals access housing, medical care, community services, and independence. The role emphasizes person-centered assessments, ISP development, and advocacy to empower participants and families.

Qualifications

• Bachelor's degree in social work, Psychology, Human Services, or related field.
• No experience required!
• Strong organizational, communication, and advocacy skills.

Responsibilities

• Conduct person-centered assessments to identify needs, preferences, and goals.
• Develop, implement, and update Individual Support Plans (ISPs).
• Provide participants with choice and advocacy in providers and care options.
• Coordinate with medical professionals, behavioral specialists, family, and community resources.
• Monitor and evaluate services for safety, satisfaction, and progress toward independence.
• Support individuals with disabilities and aging population in accessing community resources.
• Maintain documentation, confidentiality, and compliance with standards.
• Participate in ongoing training and professional development.
