Qualifications

• Bachelor's degree in Social Work, Psychology, Human Services, or related field.
• Strong organizational, communication, and advocacy skills.
• Comfort traveling up to 50% to meet with participants in homes and communities.
• Valid driver's license, auto insurance, and reliable transportation.
• Commitment to maintaining required certifications and state clearances.

Responsibilities

• Conduct person-centered assessments to identify participants' needs, preferences, and goals.
• Develop, implement, and update Individual Support Plans to ensure services meet needs.
• Provide participants with choice and advocacy in providers and care options.
• Coordinate with medical professionals, family, and community resources to deliver quality care.
• Monitor and evaluate services for safety, satisfaction, and progress toward independence.
• Assist both individuals with disabilities and aging population in accessing resources.
• Maintain documentation, confidentiality, and program compliance.
• Participate in ongoing training and professional development.
