Job summary

Care Lync is seeking a dedicated Support Coordinator/Service Coordinator/Case Manager to develop individualized care plans, coordinate services, and monitor outcomes for adults with disabilities. The role involves identifying needs for service delivery and ensuring appropriate resources are in place to support independent living across home and community settings.

Qualifications

• Experience with Microsoft Office and state-maintained databases.
• Maintain CPR/First Aid certification and updated state-related clearances.
• Complete and maintain required trainings and certifications.

Responsibilities

• Complete assessments to identify participant needs and desires for service delivery.
• Develop and modify a Person-Centered Service Plan for each participant.
