Job summary

Care Lync is a social services agency providing Support Coordination for adults with IDD, ASD, and disabilities enrolled in Medicaid Waiver. We offer medical/dental/vision, 401k, and PTO in a cooperative environment.

The Support Coordinator develops individualized care plans, coordinates services, and monitors outcomes to ensure professional, cost-effective care and independence in home and community settings.

Qualifications

• BS/BA degree in related course work.
• Experience with Microsoft Office and state-maintained databases.
• Maintain state related Clearances and required trainings.
• Valid driver's license and vehicle insurance, registration on file with Care Lync.
• Complete required trainings as regulations may change.

Responsibilities

• Complete assessments to identify participant needs and desires for service delivery.
• Develop a Person-Centered Service Plan and update annually or with changes.
• Ensure participant choice of providers and access to ISP-supported services.
• Coordinate with care providers to ensure seamless physical and behavioral care.
• Travel to participants up to 50% as needed.
