Job summary

Care Lync is a Social Services agency focused on providing Support Coordination services to adults with IDD, Autism Spectrum Disorder, and physical disabilities enrolled in Medicaid Waiver. We help individuals navigate and access services to live independently in their home and community.

The Support Coordinator develops individualized care plans, coordinates services, and monitors outcomes to ensure professional, comprehensive, and cost-effective support.

Qualifications

• BS/BA degree in related course work.
• Experience with Microsoft Office, state-maintained databases.
• Must maintain proficiency in company sponsored training and certifications.
• Maintain CPR/First Aid certification and updated state related Clearances.
• Successfully complete and maintain training courses as required or amended by program regulations.

Responsibilities

• Complete assessments to identify participant needs and desires for service delivery.
• Develop a Person-Centered Service Plan for each participant and modify annually or with significant changes.
• Ensure participant choice of providers by providing information for informed decisions.
• Ensure Behavioral Support Plan and Crisis Plan align with the ISP.
• Report incidents of abuse, neglect or exploitation to supervisor.
• Coordinate with other care coordinators to ensure seamless service delivery.
• Research and develop alternative solutions to participant needs.
• Maintain confidentiality of sensitive information and participate in required trainings.
• Consult with supervisors to account for participant needs and limitations.
• Travel to participants as needed (approximately 50%).
