Care Lync in the United States is seeking a dedicated Support Coordinator / Service Coordinator / Case Manager to develop individualized care plans and broker services to adults with developmental and physical disabilities.

You will coordinate housing, medical, and community supports, monitor outcomes, and ensure privacy. Travel may be required and you will work within a team to help clients live independently.

Qualificações

• BS/BA degree in related course work.
• Experience with Microsoft Office and state-maintained databases.
• Maintain CPR/First Aid certification and updated state related Clearances.
• Valid Driver's License including personal vehicle insurance coverage for liability and current vehicle registration.
• Inform supervisor immediately when any change in auto insurance, policy, or license status.

Responsabilidades

• Complete assessments to identify participant needs and desires for service delivery.
• Develop a Person-Centered Service Plan for each participant and review annually or with major changes.
• Ensure participant choice of providers by giving information for informed decisions.
• Ensure Behavioral Support Plan and Crisis Plan align with ISP.
• Coordinate with other care coordinators to ensure seamless service delivery.
• Research and develop alternative solutions to participant needs.
• Maintain confidentiality of sensitive health information.
• Attend trainings and recertifications as required.
• Regularly consult with supervisors to consider participant needs and limitations.
• Travel up to 50% to participant homes as needed.
