Job summary

Care Lync seeks a dedicated Support Coordinator / Service Coordinator / Case Manager to develop individualized care plans, broker services, and monitor outcomes for adults with developmental and physical disabilities. You will coordinate housing, medical, social, and community supports, ensuring cost-effective, client-centered care.

The role involves travel up to 50% and requires collaboration with supervisors and care teams to optimize participant outcomes in home and community settings.

Qualifications

• Bachelor’s degree or higher preferred.
• Experience with Microsoft Office and state-maintained databases.
• Willingness to complete and maintain required training and certifications.

Responsibilities

• Complete assessments to identify participant needs and desires for service delivery.
• Develop a Person-Centered Service Plan and update annually or as changes occur.
• Coordinate with providers to ensure informed participant choices.
