Job summary

Care Lync is a Social Services agency focused on providing Support Coordination (Service Coordination/Case Management) to adults with Intellectual, Developmental and Physical Disabilities including Autism Spectrum Disorder enrolled in the Medicaid Waiver program. Our Support Coordinators help individuals navigate and secure services to live independently in home and community.

Qualifications

• BS/BA degree in related field.
• Minimum 3 years related experience providing case management for individuals with IDD, Autism Spectrum Disorder, or Physical Disabilities.
• Experience with Microsoft Office, state-maintained databases.
• Maintain CPR/First Aid certification and updated state related Clearances.
• Valid Driver's License including personal vehicle insurance coverage for liability and current vehicle registration.

Responsibilities

• Complete assessments to identify participant needs and desires for service delivery.
• Develop a Person-Centered Service Plan for each participant and modify annually or with significant changes.
• Ensure participant choice of providers by providing information for informed decisions.
• Coordinate with other care coordinators to ensure seamless service delivery.
• Ensure confidentiality regarding sensitive material and private health information.
• Attend required trainings and recertification classes.
• Log and report incidents of abuse, neglect or exploitation promptly.

Education

BS/BA degree in related field
