Job summary

Care Lync is a Social Services agency providing Support Coordination to adults with IDD, ASD, and other disabilities under the Medicaid Waiver program. We assist clients in navigating and securing needed services to live independently at home and in the community.

The role includes developing individualized care plans, brokering services, monitoring outcomes, and coordinating with other care providers. Travel to participants is required up to 50%.

Qualifications

• BS/BA degree in related course work.
• Experience with Microsoft Office and state-maintained databases.
• Maintain CPR/First Aid certification and updated state related Clearances.
• Valid Driver's License including personal vehicle insurance coverage.
• Willingness to travel up to 50%

Responsibilities

• Complete assessments to identify participant needs and desires for service delivery.
• Develops a Person-Centered Service Plan and modify annually or with major changes.
• Ensure participant choice of providers with information for fully informed decisions.
• Ensure Behavioral/ Crisis Plans align with the ISP.
• Coordinate with other care coordinators to ensure seamless services.
• Travel to participants as needed (50% travel)
