Job summary

Care Lync is a Social Services agency focused on providing Support Coordination services to adults with IDD, ASD, and physical disabilities enrolled in Medicaid Waiver. We help clients navigate and secure services to live independently at home and in the community.

The Support Coordinator develops individualized care management plans, implements the plan through coordination of services, and monitors outcomes to ensure professional, comprehensive, and cost-effective care.

Qualifications

• BS/BA degree in related coursework.
• Experience with Microsoft Office, state-maintained databases.
• Must maintain proficiency in company sponsored training and certifications.
• Maintain CPR/First Aid certification and updated state related Clearances.
• Successfully complete and maintain training courses as required by program regulations.
• Valid Driver's License including personal vehicle insurance and current vehicle registration.

Responsibilities

• Complete assessments to identify participant needs and desires for service delivery.
• Develop a Person-Centered Service Plan and update annually or with significant changes.
• Ensure participant choice of providers with information for informed decisions.
• Ensure Behavioral and Crisis Plans align with the ISP.
• Report incidents of abuse, neglect, or exploitation immediately.
• Actively coordinate with other care coordinators to ensure seamless service delivery.
• Research and develop alternative solutions to participant needs.
• Maintain confidentiality of sensitive information.
• Attend required trainings and recertifications.
• Communicate with supervisors to consider participant needs and limitations.
• Travel up to 50% to participants as needed.
