Job summary

Care Lync is a Social Services agency providing Support Coordination and Case Management for adults with IDD, Autism, and Disabilities enrolled in Medicaid Waiver. We enable independence through personalized plans, coordination of services, and ongoing monitoring, with a strong focus on client-driven goals.

Our team conducts comprehensive assessments, develops individualized care plans, and coordinates providers to ensure cost-effective, quality outcomes.

Qualifications

• BS/BA degree in related coursework.
• Experience with Microsoft Office and state-maintained databases.
• Maintain CPR/First Aid certification and state related clearances.
• Valid Driver's License including vehicle insurance and registration.
• Complete trainings and certifications as required by program regulations.
• Inform supervisor immediately when auto insurance or license status changes.

Responsibilities

• Complete assessments to identify participant needs and desires for service delivery.
• Develop a Person-Centered Service Plan for each participant and update annually or when changes occur.
• Ensure participant choice of providers with information for informed decisions.
• Ensure Behavioral Support Plan and Crisis Plan align with the ISP.
• Report all incidents of abuse, neglect or exploitation to supervisor.
• Actively coordinate with other care coordinators to ensure seamless services.
• Develop and research alternative solutions to participant needs.
• Maintain confidentiality of sensitive information.
• Attend required trainings and recertification classes.
• Communicate with supervisor about participant needs and abilities.
• Travel up to 50% to participant locations.
