Job summary

Care Lync is a Pennsylvania-based Social Services agency focused on Service Coordination for adults with Intellectual, Developmental and Physical Disabilities and Autism Spectrum Disorder who are enrolled in the Medicaid Waiver Program. Our coordinators help individuals live independently in their home and community.

Qualifications

• BS/BA degree in related field.
• Experience with Microsoft Office and state-maintained databases.
• Ability to perform basic arithmetic with common units.
• Maintain proficiency in company trainings and certifications.
• Clean criminal background history.
• Maintain state related Clearances.
• Successfully complete and maintain training courses as required.
• Valid Driver's License and vehicle insurance coverage; keep records on file.

Responsibilities

• Complete assessments to identify participant needs.
• Develop a Person-Centered Service Plan and update annually or with changes.
• Ensure participant choice of providers with full information.
• Ensure ISP aligns with Behavioral/Crisis Plans when applicable.
• Maintain accurate documentation and reporting.
• Report incidents of abuse, neglect, or exploitation immediately.
• Coordinate with other care coordinators for integrated care.
• Develop and maintain professional relationships with participants and families.
• Maintain confidentiality of health information.
• Attend required trainings and recertifications.
• Confer with supervisors to consider participant needs and limitations.
• Travel up to 50% to participants as needed.
