Job summary

Care Lync is a Social Services agency providing Support Coordination services to adults with Intellectual, Developmental and Physical Disabilities including Autism who are enrolled in the Medicaid Waiver Program. Our Support Coordinators work with adults to help them navigate, understand, and secure the services needed to live independently at home and in the community.

The Support Coordinator / Service Coordinator / Case Manager develops individualized care management plans, coordinates

Qualifications

• BS/BA degree in related course work.
• Minimum of 3 years related experience providing case management for individuals with disabilities.
• Experience with Microsoft Office, state-maintained databases.
• Maintain CPR/First Aid certification and updated state related Clearances.
• Valid Driver's License with personal vehicle insurance and current vehicle registration.

Responsibilities

• Complete assessments to identify participant needs and desires for service delivery.
• Develop a Person-Centered Service Plan for each participant and modify annually or with changes.
• Ensure participant choice of providers with information for informed decisions.
• Coordinate with other care coordinators to ensure seamless physical, behavioral and support services.
• Research and develop alternative solutions to participant needs.
• Maintain confidentiality of sensitive health information and participate in required trainings.
