Job summary

Qualifications

• Bachelor’s Degree in Nursing required.
• RN license required.
• 3–5+ years in geriatrics or MDS coordination preferred.
• Familiarity with RAC-CT preferred.
• Fluent English for communicating with team, residents, and families.

Responsibilities

• Coordinate assessment schedules and open resident assessments per current rules and guidelines.
• Select ARD reflecting resident condition and ensure documentation is complete and submitted to state databases.
• Meet daily with Rehab Program Manager to review therapy schedules and minimize non-scheduled assessments.
• Coordinate with Interdisciplinary Team to ensure comprehensive, timely care plans; participate in care conferences.
• Disseminate updated RAI materials to team; educate nurses and CNAs on RAI processes.
