Job summary

American Senior Communities seeks an MDS Coordinator to oversee the Resident Assessment Instrument (RAI) and support the interdisciplinary care planning process. This role emphasizes regulatory compliance and coordination across nursing teams.

Qualified candidates will have RN licensure, ideally a BSN, with experience in long-term care management and collaboration with diverse care disciplines. This is a full-time position in Indiana with comprehensive benefits.

Qualifications

• RN licensure required with preference for BSN.
• Minimum of 1 year in nursing management in long-term care.
• Two years of professional nursing in long-term, acute, restorative or geriatric settings.

Responsibilities

• Coordinate and complete the RAI and interdisciplinary care planning.
• Ensure compliance with state and federal regulatory requirements.
• Lead and promote timely, accurate RAI processes with the care team.
