Senior Lifestyle Corporation is seeking an MDS Coordinator - Skilled Nursing in Gorham, ME. The role coordinates resident assessment schedules, selects ARD, and ensures documentation meets federal and state requirements while supporting the interdisciplinary team.

Requires a Bachelor's degree and RN license, 3–5+ years of MDS experience, RAC-CT preferred. Proficiency with PointClickCare and Microsoft Office, plus HIPAA compliance and strong team collaboration are essential.

Qualifications

• Bachelor's Degree and RN license required.
• 3-5+ years of MDS experience.
• RAC-CT preferred.
• Fluency in English for communication with team and residents.
• ICD-10 competency and HIPAA awareness.

Responsibilities

• Coordinate assessment schedule and open resident assessments per federal/state rules.
• Select ARD reflecting resident condition and ensure complete documentation.
• Meet daily with Rehab Program Manager to review therapy schedules.
• Coordinate with interdisciplinary team to keep care plans current.
• Attend Care Conferences and educate team on RAI process.
• Support Managed Care case management duties as needed.
• Review state validation reports and follow up as required.
• Participate in facility committees and maintain compliance with Residents' Rights.
