American Senior Communities' Rosegate Village is seeking an experienced RN to fill the MDS Coordinator role. You will manage the Resident Assessment Instrument (RAI) and coordinate interdisciplinary care planning, ensuring compliance with state and federal regulations.

The position emphasizes clinical assessment, leadership, collaboration, and a compassionate presence for residents. BSN preferred, with at least one year in nursing management in long-term care and two years in geriatric settings.

Qualifications

• RN with BSN preferred and Indiana licensure.
• Minimum of one year in nursing management in long-term care.
• Two years of professional nursing experience in long-term/geriatric settings.
