Job summary

Lakeview Post Acute seeks a Staff Speech-Language Pathologist to evaluate residents for speech therapy needs and to develop individualized plans focused on communication and swallowing abilities. You will collaborate with physicians, nursing staff, and families to coordinate care and discharge planning.

The role involves supervising CFY Speech Clinical Fellows and students, providing staff training, and ensuring compliant documentation across the facility.

Qualifications

• Bachelor’s degree in Speech-Language Pathology; Masters/Doctorate preferred.
• Proficient with data entry in Net Health and PointClickCare.
• Ability to read technical procedures and policy manuals.
• Ability to present information and respond to questions from managers, coworkers, and families.
• Basic mathematical concepts (fractions, percentages, ratios)
• Ability to interpret written, oral, diagram, or schedule instructions.

Responsibilities

• Evaluate residents to determine speech therapy needs and rehabilitation potential.
• Develop, implement, and update individualized treatment plans.
• Supervise CFY Speech Clinical Fellows and students as applicable.
• Communicate progress and discharge planning with physicians, nursing staff, and families.
• Provide in-services and training for staff as needed.
• Maintain timely, accurate documentation per policies and payer requirements.
• Educate residents and families on goals, home exercise and safety techniques.
• Recommend durable medical equipment to facilitate independence.
