Job summary

Homestead Post Acute is seeking a Staff Speech-Language Pathologist to evaluate residents, develop treatment plans, and collaborate with the care team to support communication and swallowing abilities. The role includes supervising CFY and students, training staff, documenting services, and ensuring safety and quality in care.

Qualifications

• Bachelor's degree in Speech-Language Pathology required; master's/doctorate preferred.
• Current state license as a Speech Therapist; continuing education required.
• Proficiency with electronic health records (Net Health, Point Click Care).

Responsibilities

• Evaluate residents to determine speech therapy needs and rehabilitation potential per orders.
• Develop and update individualized treatment plans to improve communication and swallowing.
• Assist nursing with training Restorative Aides.
• Supervise CFY Speech Fellows and students in resident care.
• Communicate with physicians, staff, residents and families about progress and discharge.
• Participate in care conferences and rehabilitation meetings as needed.
• Provide in-services on training programs for facility staff.
• Maintain timely, accurate documentation per facility and payer requirements.
• Maintain safe treatment environment with infection control and safety protocols.
• Educate residents and families on goals, home exercise and safety plans.
• Recommend equipment to aid swallowing and cognition.
• Contribute to quality improvement initiatives and rehab program development.
• Relate positively with residents, families, staff, and colleagues.
