Job summary

Lakeview Post Acute seeks a Speech Language Pathologist- (Healthcare) to evaluate patients within 24 hours of referral, develop and implement treatment plans, and supervise SLP Assistants. The role requires licensure as a Speech Therapist in the state and strong documentation skills.

Responsibilities include contributing to discharge planning, educating families, and participating in care conferences with the health team.

Qualifications

• Bachelor's degree required in Speech Language Pathologist Therapy; Master's/Doctorate preferred.
• Proficient in Casamba and Point Click Care for data entry and record keeping.
• Ability to read technical procedures, policies and manuals clearly.
• Ability to present information and respond to questions from managers, coworkers and families.

Responsibilities

• Evaluate patients within 24 hours or 1 business day of referral; document delays with cause.
• Develop treatment plans and obtain physician approval for services; treat per plan.
• Supervise SLP Assistants in direct patient care and related activities.
• Record evaluations, daily progress notes, recertifications, and treatment changes as required.
