Job summary

Affinity Home Health Care Inc. is seeking a Speech Language Pathologist to deliver in-home and senior living care across Hobart, IN. You will assess, treat, and monitor communication disorders with a patient-centered approach.

The role involves coordinating with interdisciplinary teams, documenting in EMR, and providing telehealth options to extend access. A valid SLP license and CCC from ASHA are required, with opportunities for full-time, part-time, or PRN work.

Qualifications

• Valid state license or eligibility in Speech Language Pathology.
• Degree from an accredited program and current CCC from ASHA.
• Proficiency in basic computer and EMR systems.
• Current CPR certification.

Responsibilities

• Provide one-on-one speech therapy in private residences and senior communities.
• Perform evaluations, develop individualized treatment plans, and deliver evidence-based interventions.
• Support patient wellness, mobility, and independence through therapy sessions.
• Document care activities accurately in electronic medical records.

Physical & Other Requirements
