Job summary

AndHealth is seeking a Specialty Care Provider, Clinical Pharmacist in Ohio to directly work with patients within a CPA framework. You will assess therapies, optimize regimens, and educate patients on their meds and overall care plan.

You will collaborate with physicians and APPs, staying current with guidelines, and travel regionally to support partner sites. Ohio license and PharmD are required, with multi-state licensure encouraged.

Qualifications

• 3+ years direct patient care experience in outpatient pharmacy preferred.
• 2+ years of specialty pharmacy experience strongly preferred.
• PGY1 residency or equivalent work experience preferred.
• Expert knowledge of pharmacy standards of practice and laws.
• Ability to communicate clearly with providers and patients.

Responsibilities

• Provide clinical pharmacy and patient care services under CPA.
• Assess medication appropriateness, dosing, and interactions to optimize outcomes.
• Collaborate with MD/APP to adjust medication plans.
• Educate patients on medication use and health behaviors.
• Review guidelines and stay current with literature for specialty care.
• Ensure regulatory compliance and participate in QA activities.
• Travel locally/regionally to support partners and meetings.
