Qualifications

• PharmD/BS in Pharmacy or FPGEC equivalence.
• Unrestricted Ohio pharmacist license in good standing.
• Willing to obtain licenses in multiple states.

Responsabilités

• Provide direct patient care, therapeutic recommendations, and counseling for chronic conditions.
• Review patient profiles for interactions and dosage adjustments; recommend evidence-based changes.
• Collaborate with Patient Engagement to secure access via payor formularies and prior authorizations.
• Maintain URAC and ACHC documentation; participate in quality programs.
• Process, fill, verify, and dispense prescriptions accurately while balancing workflows.
• Provide on-site or cross-site remote support as needed.
• Perform other duties as assigned.
