University Hospitals seeks a Clinical Pharmacy Specialist in Primary Care for Lake County/Mentor. The role includes rounding with physicians, medication therapy management, staff education, and participating in formulary and policy development.

You will precept residents and students, contribute to safety initiatives, document clinical activities, and collaborate with local and system committees. Requires PharmD, ASHP residency preferred, Ohio license, and strong communication skills.

Qualifications

• Doctorate in Pharmacy with completion of an ASHP-accredited residency (preferred).
• Must be a Registered Pharmacist (R.Ph) in Ohio.

Responsabilités

• Provide Clinical Pharmacy Services (drug information, therapy monitoring, patient-specific consultations, staff education).
• Investigate adverse drug reactions and participate in Medication Safety and Therapeutics Committee processes.
• Precept pharmacy residents and students; educate staff; document clinical activities.
• Participate in local and system committees and workgroups relevant to the specialty.
