Job summary

Mount Carmel Health System in Ohio is seeking a Patient Care Pharmacist to provide timely, accurate, and patient-centered pharmaceutical care within the Ohio Pharmacist Practice Act. You will collaborate with a multidisciplinary team to optimize drug therapy and improve patient outcomes.

Responsibilities include coordinating medication preparation and dispensing, maintaining regulatory compliance, managing inventory, and verifying orders.

Qualifications

• Education: Bachelor or Doctor of Pharmacy required.
• Current license to practice as a Registered Pharmacist in the State of Ohio.
• PGY1 residency or three years institutional pharmacy experience in an advanced clinical setting preferred.
• Effective communication skills.
• Demonstrates the ability to plan, organize and manage pharmaceutical care, including delegating tasks to and providing direction to other members of the pharmacy team.
• Performs work independently with minimum supervision in a high-stress environment.

Responsibilities

• Coordinates the preparation, distribution, and dispensing of medication in compliance with state and federal regulations and laws; directs and assists pharmacy staff.
• Maintains regulatory readiness and records related to the pharmacy transactions and drug control.
• Manages inventory by providing information to pharmacy purchasing, removing expired stock, maintaining perpetual inventories, and processing non-formulary requests.
• Interprets, evaluates and verifies all medication orders.
• Collaborates with multidisciplinary team to optimize drug therapy and monitor for adverse drug reactions and drug interactions.
