Mount Carmel Health System is seeking a Patient Care Pharmacist to deliver safe, effective, patient-centered pharmaceutical care in alignment with our mission and values. Qualifications

• Education: Bachelor or Doctor of Pharmacy required.
• Current license to practice as a Registered Pharmacist in the State of Ohio.
• PGY1 residency or three years of institutional pharmacy experience in an advanced clinical setting preferred.
• Demonstrates the ability to plan, organize and manage pharmaceutical care, including delegating tasks to and providing direction to other members of the pharmacy team.

Responsibilities

• Coordinate preparation, distribution, and dispensing of medications in compliance with regulations and policies; direct and assist pharmacy staff.
• Maintain compliance and records related to pharmacy transactions for control and accountability of all drugs.
• Manage inventory, remove expired or contaminated stock, and process non-formulary requests.
• Interpret, evaluate, and verify all medication orders.
• Collaborate with multidisciplinary team to optimize drug therapy and monitor for adverse reactions and interactions.
