Qualifications

• Pharmacist licensed in the state(s) of practice.
• BS or PharmD from ACPE-accredited school.
• Residency or board certification in related area preferred.
• ACLS certification preferred.

Responsibilities

• Provide comprehensive pharmaceutical care to a focused population as a subject matter expert.
• Lead clinical programs to improve patient care and optimize medication therapy.
• Perform medication reviews and therapy management to ensure safe use of medications.
• Identify cost-containment efforts for medication utilization and pharmacy resources.
• Support pharmacy operations in distribution and timely medication availability.
• Collaborate with pharmacy team and other clinicians to deliver high-quality services.
• Provide education to patients and staff, and precept students and residents.
