Job summary

Ascension Living in Washington, D.C. is seeking a part-time Social Services professional for the Day Shift at Carroll Manor. You will interview patients, assess priorities, document case activity, and coordinate with teams to support treatment plans.

Qualifications include an MSW preferred, a high school diploma/GED with 2 years experience or an Associate/Bachelor’s degree, and about 3 years of experience preferred. EEO and benefits provided.

Responsibilities

• Interview patients, assess priorities and document case activity.
• Consult with managers, statutory and voluntary agencies and patient's relatives to ensure compliance with treatment plan.
• Assist clients/families with obtaining community assistance by referral to proper resources.
• Develop and maintain working relationships with community health, welfare and social agencies and seek creative means to assist the client with his/her needs.
• Assure resident safety by identifying safety hazards and emergency situations and initiates corrective action as appropriate.

What Minimum Requirements You’ll Need

Licensure / Certification / Registration:

• Master Social Worker, preferred.

Education:

• High school diploma/GED with 2 years of experience, or Associate's degree, or Bachelor's degree required.

Work Experience:

• 3 years of experience preferred.
