Job summary

Ascension Living in Wichita, KS is seeking a Social Worker to assess participants' social, emotional, economic, and environmental needs and to develop intervention plans, with home visits possible. You will document progress and coordinate with teams to ensure continuity of care.

Requires a Master’s in Social Work and licensure; 3 years of experience preferred. This is a full-time, day-shift position within the Social Service department, offering comprehensive benefits.

Responsibilities

• Assess participants’ social, emotional, economic, and environmental needs, and develop intervention plans; home visits may be required.
• Conduct interviews, prioritize needs, and accurately document case activities and progress.
• Collaborate as a team-oriented department with managers, other departments, agencies, and caregivers to ensure alignment with treatment plans and continuity of care.
• Connect participants and caregivers with community resources, supporting discharge planning and ongoing post-care needs.

What Minimum Requirements You’ll Need

• Masters Level Social Worker.

Education:
