Job summary

Ascension Saint Joseph in Chicago, IL is seeking a Case Manager to coordinate the interdisciplinary plan of care from admission to discharge. You will advocate for resource needs and ensure appropriate post-discharge options while maintaining clear communication with the care team.

The role requires licensure/certification in Illinois (LSW/LCSW preferred) and a focus on patient-centered discharge planning, with the department dedicated to Case Management and a day shift schedule, PRN.

Qualifications

• Licensure / Certification / Registration required before hire date or transfer date.
• Clinical Counselor, Counselor or Social Worker credentials must be current and active.
• Illinois licensing requirements must be satisfied for practice in Case Management.

Responsibilities

• Coordinate the interdisciplinary care plan from admission to discharge.
• Monitor patient care via assessments and medical records, verify documentation and service appropriateness.
• Advocate for resources and post-discharge care options as needed.
• Maintain ongoing communication with supervisors and health providers to implement the health plan.

How you’ll make an impact in this role

• Coordinate the overall interdisciplinary plan of care across the continuum from admission to discharge, integrating patient/family preferences and values (LSW required, LCSW preferred).
• Monitor patient care through assessments, evaluations, and medical records, acting as a resource for adequate documentation and appropriateness of services.
• Advocate for necessary resources, the removal of barriers, and appropriate treatment options for post-discharge care.
• Maintain ongoing dialog with supervisors and other health providers to ensure the effective implementation of the health plan.

What Minimum Requirements You’ll Need

Licensure / Certification / Registration:

• One or more of the following required:
  • Clinical Counselor credentialed from the Illinois Department of Financial and Professional Regulation obtained prior to hire date or job transfer date.
  • Counselor credentialed from the Illinois Department of Financial and Professional Regulation obtained prior to hire date or job transfer date.
  • Social Worker specializing in Clinical Practice credentialed from the Illinois Department of Financial and Professional Regulation obtained prior to hire date or job transfer date.
  • Social Worker credentialed from the Illinois Department of Financial and Professional Regulation obtained prior to hire date or job transfer date.

Education

• High School diploma equivalency with 2 years of cumulative experience OR Associate's degree/Technical degree OR 4 years of applicable cumulative job specific experience required.

What Additional Preferences We're Seeking

• Previous in patient discharge experience needed
