Job summary

Ascension in Washington, DC seeks a Social Services professional for the Carroll Manor facility. The role focuses on assessing patient needs, planning interventions, and coordinating with agencies to support treatment plans.

Qualifications

• Licensure / Certification / Registration: Master Social Worker, preferred.
• Education: High school diploma/GED with 2 years of experience, or Associate's degree, or Bachelor's degree required.
• Work Experience: 3 years of experience preferred.

Responsibilities

• Investigate, assess and plan interventions to help patients cope with social, emotional, economic and environmental problems.
• Consult with team managers, statutory and voluntary agencies and patient's relatives to ensure compliance with patient's treatment plan.
• Assist clients/families with obtaining community assistance by referral to proper resources.
• Develop and maintain working relationships with community health, welfare and social agencies and seek creative means to assist the client with his/her needs.
• Assure resident safety by identifying safety hazards and emergency situations and initiates corrective action as appropriate.
