How you’ll make an impact in this role
- Investigate, assess and plan interventions to help patients cope with social, emotional, economic and environmental problems.
- Consult with team managers, statutory and voluntary agencies and patient's relatives to ensure compliance with patient's treatment plan.
- Assist clients/families with obtaining community assistance by referral to proper resources.
- Develop and maintain working relationships with community health, welfare and social agencies and seek creative means to assist the client with his/her needs.
- Assure resident safety by identifying safety hazards and emergency situations and initiates corrective action as appropriate.
Minimum Requirements
- Licensure / Certification / Registration: Master Social Worker, preferred.
- Education: High school diploma/GED with 2 years of experience, or Associate's degree, or Bachelor's degree required.
- Work Experience: 3 years of experience preferred.
