Ascension in Indianapolis, IN is seeking a healthcare professional for the Emergency Department with an evening 4x10 hour schedule (3:00PM-1:30AM). The role emphasizes coordinating care transitions, advocating for patient rights, and empowering families through education on resources.

Required education includes a Bachelor’s in Social Work; a Master’s in Social Work is also expected, with a preference for licensure in social work. At least 3 years of related experience is preferred.

How you’ll make an impact in this role

• Coordinate with interdisciplinary teams to streamline the transition from admission to discharge, ensuring a safe and continuous journey of care.


• Advocate for patient rights and preferences within the medical system to ensure the care provided is respectful and person-centered.


• Empower patients and families through education on financial and community resources, helping them build the stability needed to manage their diagnosis.

What Minimum Requirements You’ll Need

Licensure / Certification / Registration:

• One or more of the following required:




• Clinical Counselor obtained prior to hire date or job transfer date.


• Counselor obtained prior to hire date or job transfer date.


• Social Worker credentialed from the Indiana Behavioral Health and Human Services Licensing Board obtained prior to hire date or job transfer date.


• Social Worker specializing in Clinical Practice credentialed from the Indiana Behavioral Health and Human Services Licensing Board obtained within 12 Months (1 year) of hire date or job transfer date.




• Case Manager credentialed from the Commission for Case Manager Certification (CCMC) preferred.


• Case Manager credentialed from the American Case Management Association (ACMA) preferred.

Education

• Bachelor's degree in Social Work required.

Work Experience

• 3 years of experience preferred.

Additional Preferences

• Must have Social Worker license.


• Masters degree in Social Work required.
