Job summary

Ascension St. Vincent in Indianapolis, IN, is seeking an experienced Registered Nurse for Inpatient Case Management. You will lead interdisciplinary care planning from admission through discharge, coordinating transitions home or to the next level of care.

Ideal candidates hold an nursing diploma and an active RN license; CCMC certification is preferred though ACM is acceptable. Acute care inpatient experience is highly valued, with benefits and professional development available.

• Collaborate with families, clinical teams and insurers to build a unified post-discharge plan and prevent gaps in care.

How you’ll make an impact in this role

• Lead the interdisciplinary care strategy from admission to discharge to ensure every patient experiences a seamless, safe, and well-coordinated transition home or to their next level of care.
• Advocate for patient needs and personal values by removing financial or logistical barriers, ensuring they receive the necessary resources and support for a successful recovery.
• Evaluate medical necessity and care levels continuously to ensure patients receive the right treatment at the right time while optimizing hospital and insurance resources.
• Collaborate with families, clinical teams, and insurance providers to build a unified plan that prioritizes high quality outcomes and prevents gaps in post discharge care.

What Minimum Requirements You’ll Need

• Registered Nurse obtained prior to hire date or job transfer date required.
• Certified Case Manager credentialed from the Commission for Case Manager Certification (CCMC) preferred. ACM also accepted.

Education

• Diploma from an accredited school/college of nursing OR Required professional licensure at time of hire.

Additional Preferences

• Acute care Case Management experience.
• At least 2 years of acute care inpatient nursing experience required.
• Experience in Post Acute Care Coordination and acute care nursing is beneficial.
