Job summary

Saint Mary of Nazareth Hospital is seeking a part-time behavioral health professional to provide psychosocial support and case management for patients and families. You will work with an interdisciplinary team, complete assessments, and assist in discharge planning.

The role requires a related bachelor’s degree, a master’s preferred, and current board registration or internship status, plus BLS on hire. Inpatient/outpatient behavioral health experience is preferred and compensation ranges per

• Bachelor’s degree in Social Work, Marriage and Family Therapy or related field.
• Master’s in Psychology, Social Work, MFT or LPCC preferred.
• Current registration or intern status with the appropriate Board.
• BLS certification upon hire and maintained.
• Minimum one year inpatient or outpatient behavioral health experience preferred.

• Schedule: Part-Time, Monday-Friday, 10:30 am to 7:00 pm
• Hospital: Saint Mary of Nazareth Hospital
• Location: 2233 W Division St, Chicago, IL 60622
• Compensation: $28.42 to $41.17 per hour

Responsibilities

To provide emotional support, crisis intervention and counseling; information and referral; referrals for financial assistance and supportive services to patients and families with problems that are related to treatment services, illness, disability or trauma. To provide case management & milieu management, complete psychosocial assessments, assist in the development and re-evaluation of Treatment Plans and work as a member of an interdisciplinary team, and to coordinate discharge planning. Complete necessary documentation and maintain clinical record for each client during his/her length of stay.

Qualifications

EDUCATION, EXPERIENCE, TRAINING

• Bachelor’s degree Social Work, Marriage and Family Therapy or other related required.
• Masters in Psychology, Social Work, Marriage and Family Therapy, Licensed Professional Clinical Counselor or related field, preferred.
• Current registration an intern with Board, as appropriate to education and licensing requirements.
• Current BLS certificate upon hire and maintain current.
• Minimum one (1) year Inpatient l or Outpatient Behavioral Health experience preferred.
