Job summary

Ascension Living in Flint, MI is seeking a Social Service professional for the PACE Michigan facility.

This full-time day position involves interviewing and assessing participants, coordinating treatment plans with interdisciplinary teams, and referring to community resources to support health and social needs.

Qualifications

• Master's degree in Social Work required.
• 3 years of experience preferred.
• Limited or full licensure in the State of Michigan is required.

Responsibilities

• Conduct interviews and assessments to establish participant priorities and ensure accurate case documentation.
• Coordinate treatment plans with interdisciplinary teams, agencies, and caregivers to ensure compliance.
• Facilitate referrals to community resources and support programs for participants and caregivers.
• Build strong community partnerships to address participants' evolving health and social needs.
• Manage concurrent reviews and discharge planning, coordinating social services and post-discharge support.

What Minimum Requirements You’ll Need

Licensure / Certification / Registration:

Preferred Credential(s):

• Drivers License.
• Masters Level Social Worker.

Education:

• Master's degree in Social Work required.

Work Experience:

• 3 years of experience preferred.

What Additional Preferences We're Seeking

• 1-2 years of experience in case management, home care and/or clinical therapy required.
• Limited or full licensure in the State of Michigan is required.
