Qualifications

• Bachelor of Social Work required.
• Master's degree in social work or healthcare related field preferred.
• BLS Basic Life Support required.
• Licensed Social Worker in state of practice required; VA preferred.
• ACM or CCM certification preferred.

Responsibilities

• Identify and prioritize patients in need of social services using holistic biopsychosocial assessment.
• Develop and coordinate discharge plans with patient, family, and care team.
• Document plan of care clearly in patient records.
• Assist with Advance Care Planning, length of stay management and readmission prevention.
• Advocate for patients and remove barriers to timely testing and treatment.
• Provide clinical supervision to peers, social workers and students.
