Job summary

EMPACT-SPC, part of the La Frontera family, is seeking an SMI Case Manager to complete assessments and service plans for severely mentally ill consumers, coordinating care with families and providers. You will implement the ISP, monitor services, maintain records, and ensure continuity of care across teams and state agencies.

The role requires crisis-capable, patient-focused collaboration, strong documentation, and the ability to work with a multidisciplinary team. Bilingual Spanish is a plus.

Qualifications

• Must meet BHT requirements with bachelor’s degree or higher and one year of behavioral health experience.
• Or associate’s degree and two years of behavioral health experience.
• Or high school diploma and four years of behavioral health experience.
• Excellent oral and written communication skills.
• Detail oriented with strong organizational skills.
• Ability to exercise independent judgment and maintain professional boundaries.
• Maintain strict confidentiality and document events clearly.
• Strong clinical and therapeutic skills with multidisciplinary collaboration.

Responsibilities

• Complete assessment and service plans for consumers identified as Severely Mentally Ill.
• Engage with families, providers, and resources to implement the ISP.
• Monitor and modify covered behavioral and medical health services.
• Coordinate care across multiple providers and state agencies.
• Provide outreach, crisis follow-up, and transportation as needed.
• Maintain comprehensive clinical records and documentation.
• Collaborate with the consumer and family to explain treatment options.
