Job summary

La Frontera Arizona is seeking SMI Case Managers for EMPACT in the Phoenix area. The role involves performing assessments, developing ISP-based plans, and coordinating care across behavioral health and community resources to support severely mentally ill clients.

Responsibilities include collaboration with clinical teams, documenting progress, and ensuring continuity of care while meeting privacy and professional standards. A sign-on bonus may be available for a limited time.

Qualifications

• Bachelor's degree or higher in Behavioral Health or related field or equivalent experience in behavioral health.
• Demonstrated ability to work with diverse populations in crisis and community settings.
• Excellent communication and collaboration with multidisciplinary teams.

Responsibilities

• Complete assessment and service plans for Severely Mentally Ill (SMI) consumers.
• Implement services per ISP and document progress toward recovery.
• Coordinate with clinical team, family, and providers to ensure services are delivered.
• Maintain comprehensive clinical records and ensure confidentiality.
• Provide outreach, crisis follow-up, and transportation as needed.
• Collaborate with consumer, family and stakeholders to modify service plans as required.
• Ensure continuity of care across levels of care and multiple providers.
• Meet agency policies and participate in quality management activities.
